    When Melanie Miller saw that her health insurance premium payment was set to nearly triple to $914 a month this year, she stopped shopping on the Affordable Care Act marketplace.

    The 59-year-old retired teacher, who recently moved from Ohio to Michigan, now pays $341 a month for a pair of plans, one that covers routine and urgent care and another that pays fixed amounts for hospital stays. Neither meets federal standards for comprehensive coverage.

    Though she practices yoga and is healthy, Miller said she still feels “vulnerable.” If she lands in the hospital, her plan pays a flat $2,000, a fraction of the $30,000 price tag of an average hospital stay.

    “I don’t gamble. But I may as well,” she said. “This is gambling.”

    Congress’ decision late last year not to extend enhanced marketplace tax credits has boosted the appeal of alternatives to comprehensive insurance — plans like Miller’s, which have lower premiums but don’t meet ACA standards for coverage or consumer protections. Unlike plans sold on the exchanges, these options — some sold by major insurers, others by small companies or nonprofits — can deny claims with few or no legal rights for consumers to appeal. The plans are not required to cover “essential health benefits,” such as preventive care, and can impose annual or lifetime caps on benefits.

    There is debate over whether these options help or harm patients. Consumer advocates dismiss them as “junk insurance,” while proponents say restricting alternatives to pricey marketplace plans risks driving up the number of uninsured. Some states, including Kansas and Florida, and the federal government itself have eased regulations on such plans or created incentives to join them, while other states, including California and Massachusetts, have tried to deter enrollment in alternative insurance. Those regulatory guardrails, however, are now being stress-tested as premiums blow out household budgets.

    Alternative insurance takes many forms, including short-term policies, which were designed to bridge temporary gaps in coverage and often exclude preexisting conditions, and fixed-indemnity plans, which pay a flat rate per service regardless of how high costs go and are intended for supplemental use. Arrangements in which people pool their money to cover one another’s bills, including faith-based “healthcare sharing ministries,” also provide a cheaper alternative to the marketplace options. Because they are not considered insurance under federal or state law, they are not legally bound to pay for even eligible medical bills.

    Enrollment data for alternative plans is mostly confidential, but several indicators point to shifts in the market. Recent estimates suggest marketplace enrollment declined about 20% from 2025, and a KFF survey of people on the exchanges last year found that 5% switched to private, nonmarketplace individual coverage, including plans that don’t comply with the ACA. Covered California, the state’s marketplace, plans to survey former enrollees to find out where they went.

    Insurance industry insiders also report that, amid the expiration of subsidies, alternative plans are making a marketing push. Colorado insurance broker Samantha Albritton said that before ACA open enrollment, she saw more marketing from fixed-indemnity plans than in previous years. One healthcare sharing plan, Zion HealthShare, had more than 75,000 members in February — a 50% increase since last June, it said in a statement.

    Critics of these alternative plans say the major issues occur when people use them as primary insurance and don’t realize the coverage is inadequate until they need it most. “Humans have bodies that can fail them,” said Amy Killelea, an assistant research professor at Georgetown University’s Center on Health Insurance Reforms.

    Killelea and other health insurance experts say that the fine print on these plans can be difficult to parse and that enrollees don’t have the protections of traditional insurance to fall back on. A 2023 peer-reviewed study found that after reading a summary of a sample short-term policy’s benefits and a disclosure that the plan was not ACA-compliant, only half of participants understood that prescription drugs were not covered.

    When Jade Ramsey was 24, she declined insurance from her employer due to the cost of the premiums. After experiencing fatigue and unexplained bruising, she sought low-cost coverage from Southern Guaranty Insurance Company through a policy similar to a fixed-indemnity plan.

    Two weeks after enrolling, Ramsey, who lives in Arizona, was unable to walk. An emergency room visit led to a six-day hospital stay and a $143,823 bill in 2021. She was diagnosed with acute lymphoblastic leukemia. Her insurer denied coverage for this and other bills, labeling the cancer a preexisting condition and offering no other recourse after rejecting her appeal, she said.

    Those bills landed in collections, and her credit score nose-dived. Ramsey said she once visited the ER with chest pain she attributed to the stress of the six-figure debt. She eventually qualified for Medicaid, and her credit score has since recovered even though she never paid off the debt. She said collections agencies still call, but she ignores them.

    Southern Guaranty Insurance Company did not respond to requests for comment.

    Proponents of alternative insurance argue that stifling these more affordable options will just increase the ranks of those without any coverage.

    “People should be able to spend their own money financing healthcare the way that works best for them,” said Brian Blase, president of Paragon Health Institute, an influential conservative think tank. Paragon pushed for ending the enhanced marketplace tax credits, arguing they fueled improper enrollment by heightening incentives for unscrupulous brokers to sign people up without their knowledge.

    Robert Godfrey of Clearwater, Florida, appreciates having choices. When Godfrey’s monthly premium payment was slated to jump from $879 to around $1,250 this year, the 64-year-old hair salon owner switched to a $320-a-month membership with Zion HealthShare. Rarely needing medical care, Godfrey viewed the shift to a cheaper plan as a pragmatic choice. “Thank God I’m healthy,” he said.

    The Trump administration has relaxed regulations on some alternative plans. Last year, federal agencies stopped enforcing Biden-era rules on how long short-term plans could last and how they could be marketed, then offered states a marginal advantage in the competition for a share of $50 billion in federal rural health funding if they followed suit.

    In a statement, CMS spokesperson Christopher Krepich said the administration is focused on ensuring “access to affordable coverage options, strengthening competition, and reducing unnecessary regulatory burdens, while maintaining appropriate consumer protections.”

    State oversight of alternative insurance is a patchwork. In much of the nation, these plans face few restrictions. Many states, including Florida, Arizona, and Indiana, have eased limits on short-term plans in the wake of the Trump administration’s moves, allowing them to be renewed for up to three years in total.

    In Kansas, lawmakers overrode the governor’s veto to pass a bill in March providing a tax break for people who enroll in healthcare sharing ministries. In her veto, Democratic Gov. Laura Kelly warned that these ministries are unregulated, “which opens the door to all sorts of fraud and abuse.” Kansas House Speaker Daniel Hawkins countered in a news release that “House Republicans believe families should have more flexibility and more control over their healthcare decisions, not fewer options and higher costs.”

    Oklahoma weighed a similar bill earlier this year, though it did not pass.

    Not all states are friendly toward alternative plans. Over a dozen ban short-term policies or have rules restrictive enough to deter insurers from selling them. California and Massachusetts are among the states with the most stringent rules, banning short-term plans and requiring clear warnings to people considering a healthcare sharing ministry in certain circumstances. Both also tax adults who forgo comprehensive coverage, while subsidizing marketplace premiums to encourage enrollment.

    Still, the higher premiums will test these guardrails, said Héctor Hernández-Delgado, a director at the National Health Law Program, which advocates for quality healthcare for low-income people. He worries that consumers lured by the plans’ low prices could “be worse off down the road,” saddled with burdensome medical debt.

    Now in remission, Ramsey urges those considering cheaper insurance to do careful research. “Make sure it’s covering what you need to be covered,” she said. “It could be too good to be true.”

    Geneva— The World Health Organization on Wednesday said the risk of the Democratic Republic of Congo’s deadly Ebola outbreak was currently high at the national and regional levels but low worldwide.

    WHO experts said that while investigations into its origins were ongoing, given the scale of the situation in the eastern DRC, the outbreak probably began a couple of months ago.

    But the UN health agency’s emergency committee said it did not currently meet the pandemic emergency threshold.

    “WHO assesses the risk of the epidemic as high at the national and regional levels, and low at the global level,” said the organization’s chief Tedros Adhanom Ghebreyesus.

    So far, 51 cases have been confirmed in the DRC, in the eastern provinces of Ituri and North Kivu, “although we know the scale of the epidemic in DRC is much larger,” he told a press conference at the WHO’s headquarters in Geneva.

    He said Uganda had also reported two confirmed cases in the capital Kampala, including one death, while a U.S. national working in the DRC has been confirmed positive and transferred to Germany. American doctor Peter Stafford, whose family lived with him in the DRC, was evacuated to Germany and was receiving treatment, the missionary groupSerge said Tuesday.

    “There are several factors that warrant serious concern about the potential for further spread and further deaths,” said Tedros.

    “Beyond the confirmed cases, there are almost 600 suspected cases and 139 suspected deaths,” he said. “We expect those numbers to keep increasing, given the amount of time the virus was circulating before the outbreak was detected.”

    Ebola outbreak not “a pandemic emergency”

    On Sunday, Tedros declared the situation a public health emergency of international concern — the second-highest level of alarm under the legally binding International Health Regulations (IHR) — triggering emergency responses in countries worldwide.

    The WHO emergency committee convened to assess the outbreak met on Tuesday.

    “The current situation and criteria for a public health emergency of international concern have been met, and we agree that the current situation does not satisfy the criteria for a pandemic emergency,” the committee’s chair, Lucille Blumberg, told reporters from South Africa.

    Anais Legand, WHO technical officer on viral hemorrhagic fevers, said investigations were under way to pinpoint how long Ebola has been spreading in the eastern DRC.

    “Given the scale, we are thinking that it has started probably a couple of months ago, but investigations are ongoing and our priority is really to cut the transmission chain by implementing contact tracing, isolating and caring for all suspect and confirmed cases,” she said.

    WHO responds to U.S. criticism

    U.S. Secretary of State Marco Rubio on Tuesday said the WHO was “a little late” in identifying a deadly outbreak.

    President Trump, in one of his first acts on returning to office last year, set in motion a U.S. withdrawal from the WHO, which he attacked bitterly over its response to the COVID-19 pandemic. Numerous public health officials and experts warned at the time that, as one of the world’s largest funders of global health through both international and national agencies, including the WHO, America’s step back from such programs could impact efforts to provide lifesaving healthcare and combat deadly outbreaks, especially in lower-income countries.

    Asked about Rubio’s criticism, Tedros said that “maybe what the secretary said … could be from lack of understanding of how IHR work, and the responsibilities of WHO and other entities,” he said, explaining that the agency acted in support of countries rather than replacing them in outbreak responses.

    Supreme Court rulings loom in four major Trump-related cases

    WASHINGTON, May 20 (Reuters) – President Donald Trump was incensed on February 20 when the U.S. Supreme Court struck down his sweeping global tariffs, an integral part of his economic and foreign policy strategy. That may not be the last disappointment for Trump during the ​court’s current term.

    Four more major cases involving Trump are due to be decided by the top U.S. judicial body by around the end of next month. They involve his effort to ‌restrict birthright citizenship, fire a member of the U.S. Federal Reserve Board of Governors, oust a Federal Trade Commission member and end protected status for hundreds of thousands of immigrants from Haiti and Syria.

    The Supreme Court, whose 6-3 conservative majority includes three justices he appointed during his first term as president, has backed Trump in a series of decisions issued on an emergency basis since he returned to office last year. But, based on questions posed by the justices during arguments in the cases, Trump may lose on birthright citizenship and the Fed firing.

    The court ​next issues rulings on Thursday.

    “There are going to be a series of losses for the Trump administration, but I think they pale in comparison to the number of wins that the administration will get,” Georgia ​State University law professor Anthony Michael Kreis said.

    AN EXECUTIVE ORDER

    On the first day of his second term as president, Trump signed an executive order that instructed U.S. agencies ⁠not to recognize the citizenship of children born in the United States if neither parent is an American citizen or legal permanent resident, also called a “green card” holder. The directive is a central element of the Republican president’s hardline ​approach toward immigration.

    During arguments in the case on April 1, most of the justices signaled skepticism about the legality of Trump’s directive. At issue is whether Trump’s order violated citizenship language in the U.S. Constitution’s 14th Amendment as well ​as a federal law codifying birthright citizenship rights.

    None of the three Trump appointees – Brett Kavanaugh, Neil Gorsuch and Amy Coney Barrett – seemed enthusiastic about the president’s legal positions during the arguments, Kreis said.

    “The Trump administration is probably looking at a 7-2 loss,” Kreis said.

    University of Pennsylvania political science professor Rogers Smith said a majority of Americans has long supported current birthright citizenship policies.

    “There is a reason that repeated Republican efforts since the 1990s to change birthright citizenship rules have never made it out of committee in Congress, even when ​Republicans controlled both legislative chambers,” Smith said. “Most members of Congress know that most of their constituents do not favor changing current policies.”

    THE LISA COOK CASE

    Since the Fed was created in 1913, no president until Trump had tried to oust ​a central bank official. When Trump last year moved to fire Federal Reserve Governor Lisa Cook, he cited unproven claims made by one of his appointees that Cook had engaged in mortgage fraud, though she said the allegations were a mere pretext ‌to oust her ⁠over monetary policy differences.

    During January 21 arguments in the case, most of the justices expressed unease about the ramifications for the Fed’s cherished independence from political influence if they were to endorse Trump’s arguments that he acted within his powers in seeking to remove Cook.

    When the Supreme Court decided last October to hear the Cook case, it opted to leave her in the post while the legal dispute played out. It did not do so with Federal Trade Commission member Rebecca Slaughter, allowing Trump to remove her while her legal challenge proceeded.

    When the court heard arguments on December 8, the conservative justices signaled they would uphold Trump’s firing of Slaughter. The Justice Department has asked the court to use ​the case to overturn its 1935 precedent that has ​constrained presidential power by protecting the heads of independent ⁠agencies from removal.

    A 1914 law passed by Congress permits a president to oust FTC commissioners only for cause – such as inefficiency, neglect of duty or malfeasance in office – but not for policy differences to guard against political interference. Similar protections cover officials at more than two dozen other independent agencies.

    The conservative justices appeared to embrace the Trump administration’s ​arguments that such tenure protections encroached on presidential power under the Constitution.

    “A decision in favor of Trump in the Slaughter case will strip power away from Congress and ​give much more to the ⁠president,” University of Minnesota political science professor Timothy Johnson said.

    Bradley University political science professor Taraleigh Davis said that it seemed clear during the arguments in the Cook case that the justices want to protect the Fed.

    “But I didn’t hear an answer to what legal principle actually distinguishes the Fed from the FTC,” Davis said.

    TEMPORARY PROTECTED STATUS

    The court heard arguments on April 29 over the Trump administration’s move to strip 350,000 immigrants from Haiti and 6,100 from Syria of temporary humanitarian protections provided years ago ⁠by the U.S. ​government. The court traditionally has deferred to presidents on matters of immigration, national security and foreign policy.

    Trump’s administration has moved to rescind ​designations under the government’s Temporary Protected Status program that lets migrants from nations stricken by war, natural disaster or other catastrophes live and work in the United States while it is unsafe for them to return to their home countries.

    The Trump administration on Tuesday accused Hamas of using activist and Muslim Brotherhood-linked networks to support a Gaza-bound flotilla and sanctioned several organizers tied to the effort.

    The sanctions package, announced by the Treasury and State departments, also targeted activists associated with Samidoun, an anti-Israel activist network that U.S. officials describe as a front for the Popular Front for the Liberation of Palestine (PFLP), along with operatives tied to Muslim Brotherhood networks aligned with Hamas.

    The U.S. has designated the PFLP as a foreign terrorist organization. And governments including the United Arab Emirates and Saudi Arabia have designated the Muslim Brotherhood a terrorist organization. Both groups are Islamist organizations that believe in establishing Islamic theocracies where sharia, or Islamic law, is the law of the land.

    The sanctions came after the Israeli navy stopped dozens of boats from the flotilla in international waters off Cyprus and detained hundreds of activists on board. Activists involved in the flotilla say the mission is intended to deliver humanitarian aid to Gaza and challenge Israel’s blockade of the territory, while Israeli officials have described the effort as a political provocation.

    The move lands as a warning shot at a broader activist ecosystem in the U.S. that has long championed the flotilla campaign in a nexus between Islamist and pro-communist organizations, including CodePink and the People’s Forum, nonprofits funded by an American Marxist expatriate, Neville Roy Singham, living in Shanghai, promoting the agenda of the Chinese Communist Party. On Monday, CodePink co-founder Medea Benjamin published a video, protesting the “kidnapping” of flotilla members detained by Israel, calling the country a “pariah state.” Earlier this month, the People’s Forum demanded the release of other detained activists. The sanctions fit a wider Treasury Department strategy of targeting networks that use humanitarian or civil-society branding to amplify causes aligned with Hamas.

    “The pro-terror flotilla attempting to reach Gaza is a ludicrous attempt to undermine President Trump’s successful progress toward lasting peace in the region,” Treasury Secretary Scott Bessent said. “Treasury will continue to sever Hamas’ global financial support networks, no matter where in the world they are,” Bessent said.

    The move reflects growing scrutiny from U.S. officials over the role activist, nonprofit and international support networks play in amplifying or supporting groups designated as terrorist organizations.

    The sanctions freeze any U.S.-based assets of designated individuals and generally prohibit Americans and U.S.-based entities from conducting business with them.

    The State Department said the designations target three categories of what it described as Hamas enablers: organizers of a Hamas-backed flotilla attempting to reach Gaza, operatives within Hamas-aligned Muslim Brotherhood networks that facilitate violent terrorist attacks and coordinators tied to Samidoun.

    Among those sanctioned were Spain-based Saif Hashim Kamel Abukishek, a member of the Popular Conference for Palestinians Abroad who Treasury officials said served on the flotilla’s steering committee, and Jordan-based Hisham Abdallah Sulayman Abu Mahfuz, the acting secretary general and president of the Popular Conference for Palestinians Abroad.

    Treasury also sanctioned Belgium-based Samidoun coordinator Mohammed Khatib and Madrid-based activist Jaldia Abubakra Aueda, whom officials accused of helping coordinate the group’s activities in Europe.

    Treasury has previously described Samidoun as a fundraising and support platform for the PFLP in countries where the group faces legal restrictions.

    The sanctions package also targeted several operatives tied to Hamas-aligned Muslim Brotherhood networks, including members linked to Harakat Sawa’d Misr, an Egypt-based militant offshoot of the Muslim Brotherhood known by the acronym HASM.

    “Hamas uses these enablers to sustain its position in Gaza, finance its operations, and engage in terrorist violence beyond its borders,” the State Department said. “Today’s action exposes how Hamas exploits diaspora organizations, religious institutions, and purported civil society groups to advance its malign agenda while claiming humanitarian objectives.”

    Treasury accused Karim Sayed Ahmed Moghny, one of the individuals sanctioned Tuesday, of directing militant activity, training operatives in covert operations and collaborating with HASM on attacks targeting Israeli interests.

    Treasury also sanctioned Marwan Abu Ras and the Palestinian Scholars Association, which officials accused of helping align religious institutions in Gaza with Hamas’ ideology.

    The sanctions came as Israeli forces intercepted the remaining flotilla vessels Tuesday, detaining hundreds of activists aboard the boats, according to The Associated Press.

    Flotilla organizers said 428 activists from more than 40 countries were detained after Israeli forces stopped roughly 41 vessels in international waters off Cyprus, according to the AP.

    Amnesty International called for the release of flotilla activists, including Abukishek, and described the mission as a “peaceful solidarity mission” intended to break Israel’s blockade of Gaza.

    Turkey and Hamas condemned Israel’s interception of the flotilla, while Ireland’s prime minister described the operation in international waters as “absolutely unacceptable,” according to the AP.

    Samidoun told Fox News Digital it rejected the sanctions and denied having “material or organizational ties” to groups designated as terrorist organizations. The group also accused the U.S. government of politically coordinating the sanctions with Israel’s interception of the flotilla, which it described as “piracy” and the “abduction” of activists.

    Fox News Digital was unable to immediately reach representatives for Popular Conference for Palestinians Abroad through publicly available channels.
