Reza Pahlavi, the exiled crown prince of Iran, described the joint U.S.-Israeli attack on the country as promised “aid” and an act of “humanitarian intervention” by President Donald Trump.

Following the reported strikes, Pahlavi urged Iranians to abandon the regime and called on security forces to defect.

“Moments of destiny lie ahead of us,” Pahlavi wrote in a statement on social media. “Even with the arrival of this aid, the final victory will still be forged by our hands. It is we, the people of Iran, who will finish the job in this final battle. The time to return to the streets is near.”

Pahlavi declared that the Islamic Republic is collapsing.

He framed the reported strikes as assistance directed not at Iran itself, but at its ruling clerical establishment and urged the U.S. to “exercise the utmost caution” to preserve civilian lives.

“The aid that the President of the United States promised to the brave people of Iran has now arrived,” Pahlavi wrote. “This is a humanitarian intervention; and its target is the Islamic Republic, its repressive apparatus, and its machinery of slaughter — not the country and great nation of Iran.”

Pahlavi issued a blunt warning to Iran’s military, police and security services, urging them to break ranks with Supreme Leader Ayatollah Ali Khamenei.

“Now that the Islamic Republic is collapsing, my message to the country’s military, police, and security forces is clear: You have sworn an oath to protect Iran and the Iranian people — not the Islamic Republic and its leaders,” he wrote.

“Your duty is to defend the people, not a regime that has taken our homeland hostage through repression and crime. Join the people and help bring about a stable and secure transition. Otherwise, you will go down with Khamenei’s sinking ship and his regime.”

His appeal mirrored past opposition efforts to persuade Iran’s security forces to switch sides — a move that could determine whether protests spread or the regime tightens its grip.

While predicting imminent change, Pahlavi stopped short of urging immediate street demonstrations. He warned citizens to remain in their homes and stay vigilant so that when he announces an “appropriate time,” Iranians can “return to the streets for the final action.”

“We are very close to final victory,” he wrote. “I want to be by your side as soon as possible so that together we can take back and rebuild Iran.”

He also indicated he would maintain communication even if authorities moved to restrict internet or satellite access — a tactic Iranian officials have used during prior waves of unrest.

Pahlavi thanked Trump for what he characterized as support while again urging caution to avoid civilian casualties.

“I now ask you to exercise the utmost caution to preserve the lives of civilians and my compatriots,” he wrote, adding that “the people of Iran are your natural allies and those of the free world.”

Pahlavi, the son of the late Shah Mohammad Reza Pahlavi, has lived in exile since the 1979 Islamic Revolution toppled Iran’s monarchy and established the Islamic Republic.

In recent years, he has sought to position himself as a unifying opposition figure during waves of anti-regime protests, including demonstrations sparked by economic turmoil and the death of Mahsa Amini in 2022.

    • Summary
    • Trump briefings included risks, opportunities in Middle East
    • Diplomatic efforts with Iran fail to avert military confrontation
    • Iran vows retaliation, targets US and Israeli interests

WASHINGTON, Feb 28 (Reuters) – Ahead of the U.S. attack on Iran, President Donald Trump received briefings that not only delivered blunt assessments about the risk of major U.S. casualties but also touted the prospect of a generational shift in the Middle East in favor of U.S. interests, a U.S. official told Reuters.

The launch of what the Pentagon called “Operation Epic Fury” on Saturday plunged the Middle East into a new and unpredictable conflict. The U.S. and Israeli militaries struck sites across Iran, triggering retaliatory Iranian attacks against Israel and nearby Gulf Arab countries.

The official, who spoke on condition of anonymity, said the briefers described the operation to the president as a high-risk, high-reward scenario.

Trump himself appeared to echo that sentiment when he acknowledged the stakes at the onset of the operation, saying “the lives of courageous American heroes may be lost.”

“But we’re doing this not for now, we’re doing this for the future, and it is a noble mission,” Trump said in a video address announcing the start of major combat operations.

“For 47 years, the Iranian regime has chanted death to America and waged an unending campaign of bloodshed and mass murder … We’re not gonna put up with it any longer.”

The briefings from Trump’s national security team help explain how the president decided to pursue arguably the riskiest U.S. military operation since the invasion of Iraq in 2003.

Ahead of the strikes, Trump received multiple briefings from officials including CIA Director John Ratcliffe, U.S. General Dan Caine, the chairman of the Joint Chiefs of Staff, U.S. Secretary of State Marco Rubio and Defense Secretary Pete Hegseth.

On Thursday, Admiral Brad Cooper, who leads U.S. forces in the Middle East as the head of Central Command, flew to Washington to join discussions in the White House Situation Room.

A second U.S. official said that prior to the strikes, the White House had been briefed on a number of risks associated with operations against Iran, including retaliatory strikes on multiple U.S. bases in the region by Iranian missiles that could overwhelm defenses, as well as Iranian proxies attacking U.S. troops in Iraq and Syria.

The official said that despite the massive military buildup by the United States, there were limits to the air defense systems that had been rushed into the region.

Experts caution that the unfolding conflict could take dangerous turns and the first official said the Pentagon’s planning did not appear to guarantee the outcome of any conflict.

Trump called on Iranians to topple the government but that is easier said than done, said Nicole Grajewski with the Carnegie Endowment for International Peace.

“The Iranian opposition is pretty fragmented. It’s unclear what the population is willing to do in terms of rising up,” Grajewski said.

Both U.S. officials requested anonymity due to the sensitivity of the internal discussions.

The White House did not immediately respond to requests for comment. The Pentagon declined to comment.

    TRUMP’S SWEEPING GOALS

In the weeks leading up to the attack, Trump ordered a major military buildup in the Middle East. Reuters reported military planning to carry out a sustained campaign against Iran, if that is what the president chose. Plans included targeting individual officials, officials said.

An Israeli official said Iran’s Supreme Leader Ayatollah Ali Khamenei and President Masoud Pezeshkian were both targeted but the result of the strikes was not clear.

Trump made clear on Saturday that his objectives in Iran were sweeping, saying he would end the threat posed by Tehran to the United States and give Iranians a chance to topple their rulers. To accomplish this, he outlined plans to lay waste to much of Iran’s military as well as deny it the ability to build a nuclear weapon. Iran denies seeking a nuclear weapon.

“We are going to destroy their missiles and raze their missile industry to the ground… We’re going to annihilate their navy,” he said. “We’re going to ensure that the region’s terrorist proxies can no longer destabilize the region or the world and attack our forces.”

Trump’s decision demonstrates an increasing risk appetite, far greater than when he ordered U.S. special operations forces into Venezuela last month to seize that country’s president in an audacious raid.

The unfolding campaign against Iran is also riskier than when Trump ordered U.S. forces to bomb Iran’s nuclear sites in June.

Iran’s Revolutionary Guards threatened all U.S. bases and interests in the region and said Iran’s retaliation would continue until “the enemy is decisively defeated.”

Experts warn that Iran has many options for retaliation, including missile strikes but also drones and cyber warfare.

Daniel Shapiro, a former senior Pentagon official for Middle East issues, said that despite the U.S. and Israeli strikes, Tehran would still be capable of causing some pain.

“Iran has many more ballistic missiles that can reach U.S. bases than the U.S. has interceptors … some Iranian weapons will get through,” said Shapiro, also a former U.S. ambassador to Israel. “(The strikes are) a major gamble.”

In a sweeping pre-dawn bombing campaign across Iran, Israeli forces targeted sites linked to Iran’s Supreme Leader Ayatollah Ali Khamenei, a senior Israeli official confirmed to Fox News. The official said Iran’s president was also targeted as part of the joint U.S. operation, dubbed Operation Epic Fury.

Reuters reported that Khamenei was not in Tehran during the strikes and was instead transferred to a secure location.

President Donald Trump described the “massive and ongoing” operation as the opening phase of a campaign that he said would devastate Iran’s military, dismantle its nuclear program and ultimately bring about regime change.

“It will be yours to take,” Trump said in a video statement addressing the Iranian public.

Hours later, Tehran signaled it would not back down, saying it would defend itself against any attack.

“This will be probably your only chance for generations,” he added. Officials in Tehran said the country would defend itself against any attack.

Ahead of the strikes, the U.S. military amassed what Trump previously called an “armada” in Iran’s backyard. Mapped out across the Persian Gulf and beyond, the deployment tells its own story, one of calculated pressure backed by credible capability.

The buildup coincided with indirect negotiations between Washington and Tehran over Iran’s disputed nuclear program. Trump has warned that the regime must fully dismantle its nuclear infrastructure or face consequences.

THE ONLY MAP YOU NEED TO SEE TO UNDERSTAND HOW SERIOUS TRUMP IS ABOUT IRAN

At the heart of America’s force projection are the USS Abraham Lincoln and USS Gerald Ford aircraft carrier strike groups — dual mobile fortresses at sea, guarded by destroyers and equipped to unleash precision strikes at a moment’s notice.

More than a dozen other U.S. warships are also in the region to support.

For Iran, it means U.S. forces are not concentrated in a single vulnerable location — they are distributed, layered and positioned to operate from multiple directions at once.

It was not immediately clear how or when Iran might respond. But with senior leaders targeted and U.S. naval assets positioned across the region, the latest exchange marks one of the most volatile moments in the decades-long standoff between Israel, Iran and the United States.

A college student thought she had a UTI. Then came fevers, fatigue and pain: “Something’s not right”

    February 28, 2026 / 8:00 AM EST / CBS News

Emma Operacz was enjoying her summer. She was a semester away from graduating from Eastern Michigan University with a degree in psychology. She had a thriving social life, two jobs that kept her busy, and was the life of every party.

Then, in June 2024, Operacz noticed symptoms of what she thought was a urinary tract infection as she attended an old friend’s college graduation in Milwaukee. She took an over-the-counter medication and tried to ignore the annoyance in her otherwise stellar weekend.

A week later, she was still having symptoms. Prescribed antibiotics didn’t help. Operacz began to feel pain in her side and run a fever. She became so fatigued that she couldn’t work. For two weeks, she was “pretty much bedridden,” she said.

A CT scan showed no kidney stones or appendicitis. One night, Operacz “broke down crying” on the phone with her older sister Sara.

“I was like, ‘I’m not OK. Something’s not right,’” Operacz said.

Her sister immediately came and picked up Operacz from her sorority house. The next day, Operacz saw a urinary gynecologist. The doctor noticed the lymph nodes in Operacz’s groin were swollen and sent her to the emergency room.

“I finally got in to see a doctor, do an ultrasound, pee in a cup for like the 100th time,” Operacz said. “Then they did a pelvic exam. They were like, your lymph nodes are swollen. It could be from a pelvic infection, or it could be lymphoma. They just kind of threw that out there casually.”

Emma Operacz in the hospital. Emma Operacz

Operacz knew lymphoma was a kind of cancer, but wasn’t too concerned. She thought, as a healthy 21-year-old, that wasn’t “in (her) deck of cards.” But her symptoms kept getting worse. Her fever got higher and higher, and her heart became unstable. After a week in the hospital, Operacz underwent a biopsy.

Before the anesthesia had even worn off, doctors had a diagnosis: Stage IV lymphoma.

“I watched my sister disappear in front of me”

Operacz was diagnosed with T-cell lymphoma, a form of non-Hodgkin’s lymphoma. The disease affects about 15% of non-Hodgkin patients, said Dr. Eric Jacobsen, an oncologist and the clinical director of the adult lymphoma program at the Dana-Farber Cancer Institute. Her specific cancer was ALK+ anaplastic large cell lymphoma, which is more likely to affect younger patients.

The diagnosis gave doctors some answers. Operacz was stabilized and eventually released from the hospital. Things seemed manageable, Sara Operacz said. But two days later, Operacz was in so much pain she couldn’t move. Her lymph nodes “felt like they exploded everywhere.” Her family called an ambulance. Doctors began administering chemotherapy as soon as she arrived at the hospital.

Operacz was treated and discharged again, but infections, migraines and other ailments kept bringing her back. She said her life felt like it had been “put on pause.” She “couldn’t stay out” of the hospital. In one incident, Sara Operacz had to call an ambulance after her younger sister “collapsed right there in my apartment.”

After a few weeks, new scans showed the cancer had spread to her central nervous system and brain. Her area hospital could no longer treat her. She was transferred to the Cleveland Clinic on July 12, 2024. Operacz said she was barely lucid during her time there. Treatments kept failing, and her odds of survival kept falling.

“I watched my sister disappear in front of me. Emma went from this vibrant 21-year-old to someone who was 80 pounds, skin and bone. I had to help her bathe. I had to be there for things no sibling should have to do,” Sara Operacz said. “Hope kept shrinking. At one point, I was planning her funeral. I helped her sign a will. I was very certain I was going to lose my sister.”

An experimental treatment and bone marrow transplant

Dr. Deepa Jagadeesh, Operacz’s oncologist, had one last suggestion. Small studies had shown a lung cancer medication called alectinib could help pediatric patients with the same cancer Operacz had. Unlike most medicines, it could cross the blood-brain barrier, making it more effective for treating the brain and central nervous system. Jagadeesh received permission from Operacz’s health insurance to prescribe the medication and began administering it on August 20, 2024.

Operacz’s condition quickly improved, Jagadeesh said. She was able to recover at an Airbnb her parents had rented in Cleveland. By September, she was in remission. But her medical journey wasn’t over yet: Jagadeesh wanted to perform a bone marrow transplant.

The chemotherapy that precludes the treatment can eradicate “any microscopic areas of lymphoma” that remain, and the stem cells in the donated bone marrow can “essentially attack the lymphoma the way it would attack a virus or bacteria,” Jacobsen explained. It can help a patient who is in remission stay healthy.

Sara Operacz was a match to donate. She said she had “no hesitation” about the process.

“I’m the oldest sibling, and being the only match felt like this was meant to be — like it was my responsibility. There was zero doubt in my mind. Not a single moment of fear or regret,” Operacz said. “I would do it again in a heartbeat. I would do it a 100 times over if it meant saving her.”

The transplant was conducted on Nov. 8, 2024, one day after Operacz’s 22nd birthday. Afterwards, Operacz spent another 70 days in Cleveland. She had to stay in isolation as her immune system recovered.

“I was far away from all my friends; I had nothing to do. I was over being with my mom and dad. It was winter. It was just a dark time,” Operacz said. “It was like ‘Alright, everything’s over now. I beat the cancer. I did the transplant. What’s next? How am I supposed to start rebuilding my life?’”

    “Building a new Emma”

Operacz took online classes while recovering. After her isolation ended, she was able to take some short road trips. Slowly, she began to feel more like herself. She traveled with friends and moved to Ohio with Sara. In December 2025, she graduated at last.

“I was able to say ‘goodbye’ to college Emma. I was able to actually build a new me, and it felt like a fresh start,” Operacz said.

Operacz will continue to receive regular follow-up care. Jagadeesh said that two years after a bone marrow transplant, a patient’s risk of relapse goes down. After five years, patients are considered cured.

Operacz isn’t waiting around for that milestone. In January, she started a graduate program in social work. She said her goal is to work with cancer patients.

“I felt like I could resonate with it and helping people would be like my way of giving back, because of all the help I received throughout my treatment,” Operacz said. “I realized that, yeah, I miss the old me, but I can’t hold on to that. I just gotta focus on building a new Emma.”
